Care Management Solutions was established in 1991 by two Master’s prepared Social Workers with a vision
of quality services and dignified care for elderly persons and their families. As medical Social
Workers for many years, Dianne Boazman and Betty Landreaux saw first-hand the fragmentation of health
services and gaps in needed community resources, resulting in frustration for families and many unmet
needs felt by their loved ones.
Dianne and Betty then became aware of a growing organization of professional Geriatric Care
Managers with similar goals and values. The National Association of Professional Geriatric Care
Managers was founded in 1986 by a cutting-edge group of clinical social workers, nurses and
gerontologists who shared a mission of promoting high quality, individually-tailored care for the

By 1991, Dianne and Betty formed Care Management Solutions
and became active members of their national association.
They have each made significant contributions to NAPGCM over
the last 14 years, both on a national level and in their
regional chapter.
As the company has evolved over the years, services have expanded in response to requests from
professionals, as well as the changing needs of client families. Personalized consultation and
recommendations to families remain the cornerstone of the company’s care management service.
Additional services have been expanded to cover broader areas, including court appointed
guardianships, bill paying services and caregiver payroll services.
After 10 years of providing comprehensive geriatric care
management services for elderly and severely disabled persons,
client experiences and recognition of gaps in caregiver services
led to the creation of a new division – Home Care Solutions,
an innovative approach to specialized sitter/companion services
for individuals with Alzheimer’s Disease.
